CNA Steps to Apply

Admission Procedure

  1. Complete CNA Application and Checklist Part 1
    1. Includes applying to DACC, sending in official transcripts, and scheduling a meeting with the CNA Program Coordinator.
    2. For detailed instructions, please refer to Part 1 of the actual CNA Application and Checklist.
  2. Complete CNA Application and Checklist Part 2
    1. Includes completion of fingerprinting, health physical, TB test(s), and drug test.
    2. You will not receive Part 2 of the Application and Checklist until you have completed Part 1.
  3. Once Part 1 and Part 2 are completed and negative test results are obtained and submitted, then your application is considered complete.
  4. Successful completion of all steps will place students into consideration for the first available class.

Program Completion

  1. Must complete NURS 110 (classroom portion) of class with 82% grade or higher in order to continue to clinicals.
  2. Must pass clinical with a “Satisfactory” grade to complete the entire program and be eligible to sit for the Illinois Nurse Aide Certification Exam.
