Certified Nurse Assistant Admission Procedure

Applicants Must Complete These Steps In The Following Order

  1. Fill out DACC Application if not already a DACC Student
    1. Click "Apply" on the DACC Homepage
  2. Have your official high school transcript sent from your high school to records@dacc.edu
    1. Must include:
      1. Date of graduation
        1. If you are a high school student, have your high school send your most recent transcript showing Math and English grades
      2. GPA
      3. Class rank
  3. Complete TOEFL exam if English is not your first language.

  4. Complete CNA Application Online
    1. Will be available online on the DACC CNA webpage when applications are open
    2. Must have your DACC Student ID Number available
    3. must have a state issued photo ID such as Illinois State Driver's License, Illinois State ID card, or U.S. Passport
    4. Must have your social security card
  5. Schedule your appointment with the CNA Program Coordinator
    1. You will receive a link to schedule your appointment once your CNA Application is reviewed and is complete.
  6. Attend the meeting with the CNA Program Coordinator
    1. Mandatory Items to bring to your appointment:
      1. State issued photo ID
        1. Must be the actual card
        2. Must be one of the following:
          1. Illinois State Driver's License
          2. Illinois State ID card or
          3. U.S. Passport
      2. Social Security Card
        1. Must be the actual card
    2. If you do not have a State Issued Photo ID or your actual Social Security Card, you must obtain these prior to meeting with the CNA Program Coordinator
  7. Complete second part of the application process by the due date
    1. Includes completion of fingerprinting, health physical, TB testing, and drug screening.
    2. You will receive all of the required documents at the meeting with the CNA Program Coordinator.
  8. Your CNA application is complete once all documents are returned and test results are satisfactory.
  9. Successful completion of all steps will place students into consideration for the first available class.
