Applicants Must Complete These Steps In The Following Order
- Fill out DACC Application if not already a DACC Student
- Click "Apply" on the DACC Homepage
- Have your official high school transcript sent from your high school to records@dacc.edu
- Must include:
- Date of graduation
- If you are a high school student, have your high school send your most recent transcript showing Math and English grades
- GPA
- Class rank
- Complete TOEFL exam if English is not your first language.

- Complete CNA Application Online
- Will be available online on the DACC CNA webpage when applications are open
- Must have your DACC Student ID Number available
- must have a state issued photo ID such as Illinois State Driver's License, Illinois State ID card, or U.S. Passport
- Must have your social security card
- Schedule your appointment with the CNA Program Coordinator
- You will receive a link to schedule your appointment once your CNA Application is reviewed and is complete.
- Attend the meeting with the CNA Program Coordinator
- Mandatory Items to bring to your appointment:
- State issued photo ID
- Must be the actual card
- Must be one of the following:
- Illinois State Driver's License
- Illinois State ID card or
- U.S. Passport
- Social Security Card
- Must be the actual card
- If you do not have a State Issued Photo ID or your actual Social Security Card, you must obtain these prior to meeting with the CNA Program Coordinator
- Complete second part of the application process by the due date
- Includes completion of fingerprinting, health physical, TB testing, and drug screening.
- You will receive all of the required documents at the meeting with the CNA Program Coordinator.
- Your CNA application is complete once all documents are returned and test results are satisfactory.
- Successful completion of all steps will place students into consideration for the first available class.
